Forms used by Cuala
Requirements to Coach a Cuala Team
This form is to be completed by club members that wish to take on the role of ‘Coach’ or ‘Mentor’.
The completed form will be sent to the Club Secretary/ Children’s Officer to be checked/matched with the member’s vetting , Safeguarding and coaching award status.
All GAA coaches must hold current Vetting (1) by An Garda, hold a current GAA Safeguarding certificate (2) and hold a minimum coaching qualification (3) to coach underage players.
Full compliance with the statutory requirements (1 & 2 above) and the GAA coaching (3) requirement must be in place before a member can take up the role of coach/mentor with a Cuala underage team.
Code of Behaviour Declaration
This declaration is to be completed by club members that wish to take on the role of ‘Coach’ or ‘Mentor’ with an underage team.
The completed form will be sent to the Club Secretary/ Children’s Officer – it will be considered as a formal confirmation by the member that they have read the GAA Code of Behaviour , understood the implications for taking on the role, and, agree to be bound by the principles set out in the Code when participating, playing or attending our Gaelic Games.
Incident/Accident Report Form
This form is to be completed by club members that witness an accident/incident at a Cuala organised activity or event.
The completed form will be copied to the Club Secretary/ Children’s Office to be actioned.
GYM Access Request
Pitch Maintenance & Repair Request
This form is to be completed by club members that identify worn or damaged areas on the DLR pitches used by Cuala.
The completed form will be copied to the Cuala Office/ Cuala’s DLR Pitch Liaison person to be actioned.
